Health Treat Secure Inc. ("Health Treat Secure") is committed to protecting your personal and health information. This Privacy Policy explains our data practices when you use our platform.

1. Personal Information Collection

We collect name, email, phone, address, date of birth, gender, SSN (insurance), and emergency contacts during account creation, booking, and enrollment.

2. Protected Health Information

PHI including medical history, symptoms, diagnoses, prescriptions, lab results, treatment plans, and allergy information shared during consultations — all HIPAA-compliant.

3. Financial and Payment Data

Payment cards, bank accounts, insurance policy/group numbers, billing addresses. Card data processed through PCI DSS Level 1 processors, never stored on our servers.

4. Technical Data Collection

IP address, browser type, OS, device identifiers, pages visited, time on page, click patterns, referring URLs, screen resolution, and telehealth/booking interaction data.

5. Cookies and Tracking

Essential cookies for auth/sessions, functional for preferences, analytics for usage analysis. No cookie data sold to advertisers. Manage via browser or consent tool.

6. Data Usage Purposes

Deliver healthcare, schedule appointments, facilitate telehealth, process payments/insurance claims, send reminders/results/follow-ups, personalize recommendations, improve platform, comply with law, detect fraud.

7. Appointment Communications

Confirmations, reminders via email/SMS, pre-visit instructions, post-visit summaries, prescription notifications, follow-up care. Essential transactional communications.

8. Insurance Data Sharing

Shared with carriers, clearinghouses, billing partners for eligibility verification, claim submission, pre-authorizations, and reimbursements per HIPAA TPO provisions.

9. Promotional Communications

With explicit consent: new services, wellness programs, campaigns, partnerships, offers. Opt out anytime via unsubscribe link or account preferences without affecting transactional emails.

10. Service Provider Sharing

Cloud hosting (HIPAA-eligible AWS), payment processing (Stripe, PCI DSS L1), telehealth (Zoom Healthcare), analytics (anonymized), email (SendGrid). All bound by DPAs.

11. Provider Network Access

PHI shared with licensed providers involved in your care, each bound by HIPAA, confidentiality obligations, and BAAs. Minimum necessary access only.

12. Legal Disclosures

Court orders, subpoenas, government requests; suspected child abuse reporting; public health reporting; health oversight audits; law enforcement (imminent harm); workers' compensation.

13. Business Transfers

Mergers, acquisitions, reorganizations, bankruptcies, asset sales may transfer your data. Email notification before different privacy policy applies.

14. Encryption at Rest

AES-256 (FIPS 197) for all data at rest. Encrypted database connections. Separate key management for backups. Key rotation per best practices.

15. Encryption in Transit

TLS 1.3 with perfect forward secrecy for all data in transit. HTTPS-only API endpoints. Certificate pinning for mobile apps.

16. Infrastructure Security

HIPAA-eligible cloud: dedicated VPCs, encrypted storage, IAM, security groups, WAF, IDS, automated scanning. SOC 2 Type II certified.

17. Access Control

MFA, 12+ character passwords, 15-min session timeout, IP allowlisting for admin, role-based access. Immutable audit trail for all access.

18. Personnel Security

Background checks, confidentiality agreements, HIPAA training (hire + annual), role-limited data access. Vendor personnel subject to equivalent BAAs.

19. Security Audits

Annual penetration testing, quarterly vulnerability assessments, continuous monitoring, semi-annual incident response plan testing, annual SOC 2 Type II audit.

20. Data Retention

Active: while active. Health records: per state law (7–10 years). Payment: 7 years. Marketing: until unsubscribe/deletion. Inactive: anonymized after 3 years.

21. Right to Access

Request copy of all data. Processed within 30 days per HIPAA. Provided electronically via portal or written request to Privacy Officer.

22. Right to Correction

Request correction of inaccurate data. Health record corrections coordinated with treating provider. Via portal or privacy@healthtreatsecure.info.

23. Right to Deletion

Request deletion subject to legal retention. HIPAA records restricted (not deleted) during mandatory period. Non-essential data erased within 45 days.

24. Right to Portability

Request data in structured format (JSON/CSV). Health records exportable as CCD compatible with any EHR system.

25. Right to Restrict Processing

Restrict PHI disclosures for payment/operations when paid out-of-pocket. Accommodate reasonable requests per HIPAA.

26. Right to Opt Out

Opt out of promotional emails, SMS, push anytime without affecting healthcare access or transactional communications. Processed within 10 business days.

27. HIPAA NPP

Separate Notice of Privacy Practices detailing rights, PHI use/disclosure, legal duties. Available in portal and at first service.

28. State Privacy Rights

CCPA/CPRA (California), Virginia CDPA, Colorado CPA, Connecticut CTDPA, and other state rights honored.

29. Children's Privacy

Not for under-13. No knowingly collected under-13 data. Minors 13–17 require parental/guardian consent.

30. Breach Notification

PHI breach: notify within 60 days per HIPAA. Include description, data types, protective steps, mitigation. Via email or first-class mail.

31. Policy Updates

Material changes: email 30 days before effectiveness. "Last updated" revised. Continued use constitutes acceptance.

These Terms govern your use of Health Treat Secure website, mobile applications, and healthcare services. By using our Services, you accept these Terms.

1. Agreement Acceptance

Creating an account, booking, purchasing insurance, or using Services confirms you are 18+ (or have parental consent), legally competent, and accept these Terms as binding.

2. Service Description

Digital healthcare platform connecting patients with licensed providers: primary care, dermatology, dental, haircare, weight loss, fitness, health/life insurance brokerage. Availability varies by state.

3. Eligibility

US resident, 18+ (or 13 with consent), not prohibited from healthcare, capable of providing accurate information.

4. Account Registration

Account required for full access. Responsible for credential security, all activities, prompt unauthorized access reporting. Two-factor authentication recommended.

5. Information Accuracy

All information — personal, medical, insurance, payment — must be accurate and current. False information risks misdiagnosis, inappropriate treatment, claim denial, and suspension.

6. Booking Process

Booking is a request; confirmation via email/SMS once finalized. Times in provider's time zone unless specified.

7. Patient Cancellation

Free cancellation/rescheduling 24+ hours before. Within 24 hours: fee up to $50 (disclosed at booking). Repeated late cancellations restrict booking.

8. Provider Cancellation

Immediate notification, earliest alternative offered, no charge. Pre-payments refunded in full within 5–7 business days if no alternative.

9. No-Show Policy

Missed appointment without canceling: fee up to $75. Three no-shows within 90 days may suspend account.

10. Rescheduling

Free 24+ hours before. Within 24 hours = late cancellation. Subject to availability.

11. Pricing

Displayed before booking. Varies by service, provider, format, location. USD inclusive of taxes unless stated. 30-day update notice.

12. Payment Methods

Visa, Mastercard, Amex, Discover, debit, HSA/FSA, Apple Pay, Google Pay. PCI DSS L1 processing at booking or service conclusion.

13. Insurance Billing

Direct billing with valid insurance. You owe co-pays, co-insurance, deductibles, non-covered charges. If denied, full fee owed. Not responsible for carrier decisions.

14. Late/Failed Payments

Due at service. Overdue: 1.5%/month (18% annually). Failed: $25 fee. 60+ days overdue: services suspended.

15. Refunds

Provider cancellations, quality issues, duplicates. Others case-by-case. Approved: 5–10 days to original method. Completed consultations non-refundable.

16. Health Insurance Brokerage

Licensed broker, not insurer. Present plans, facilitate enrollment, assist claims. Terms by carriers. Commissions disclosed, don't affect premium.

17. Life Insurance Brokerage

Licensed broker. Present options, assist applications/underwriting, provide support. Coverage/premiums by issuing carriers.

18. Liability Limitation

Total aggregate liability ≤ fees paid in preceding 12 months. No indirect, incidental, special, consequential, or punitive damages.

19. Warranty Disclaimer

"As is" without merchantability, fitness, non-infringement, or uninterrupted operation warranties. No availability/error-free guarantee.

20. Healthcare Disclaimer

We connect patients with providers; don't provide direct medical services. Clinical care by independent, licensed practitioners. No guaranteed outcomes.

21. Emergency Disclaimer

Not for emergencies. Life-threatening: call 911 or visit nearest ER. Not liable for harm from failure to seek emergency care.

22. Telehealth Terms

Must be in a state where provider is licensed; stable internet; compatible device; privacy during consultation; in-person follow-up may be recommended.

23. Intellectual Property

All content, design, software, code are our property/licitors', protected by US IP law. No reproduction or derivatives without written consent.

24. User Conduct

No unlawful use; false info; impersonation; platform disruption; unauthorized access; harassment; unauthorized recording; automated scraping.

25. Privacy Integration

Use governed by our Privacy Policy, incorporated by reference. Consent to data collection/use including PHI handling under HIPAA.

26. Indemnification

You indemnify Health Treat Secure, officers, directors, employees, agents, service providers from claims, liabilities, damages, costs, expenses from your use or Terms violations.

27. Force Majeure

Not liable for circumstances beyond reasonable control: disasters, pandemics, government actions, wars, terrorism, labor disputes, power outages, internet disruptions.

28. Dispute Resolution

Internal via support@healthtreatsecure.info first. Unresolved: AAA binding arbitration in Chicago, IL. Waive class-action and class-wide arbitration.

29. Governing Law

USA law without conflict-of-law provisions. Non-arbitration actions in Cook County courts. Consent to personal jurisdiction.

30. Modifications

May modify anytime. Material changes: email 30 days before effectiveness. Continued use = acceptance.

31. Severability

Unenforceable provision limited to minimum necessary. Remainder in full force.

32. Entire Agreement

These Terms, Privacy Policy, HIPAA NPP, and service agreements constitute the entire agreement between you and Health Treat Secure.

3. Promotional emails stop: Health tips, campaigns, new services, offers, partner emails, surveys, events, recommendations.

4. Processing: Within 10 business days. Some queued emails may arrive during processing.

5. Confirmation: Single confirmation email (transactional) once processed.

6. Re-subscribe: Anytime via account preferences or newsletter form.

7. Partial preferences: Weekly digest, monthly digest, specific topics, or announcements only in account settings.

8. Partners: Unsubscribing from Health Treat Secure doesn't affect carrier/partner communications. Unsubscribe each separately.

9. Data retention: Retained per Privacy Policy/HIPAA. Email moved to suppression list.

10. Account active: Full access to appointments, telehealth, prescriptions, insurance continues.

11. SMS: Text reminders are transactional. Opt out by replying STOP.

12. Push notifications: Managed in device Settings or app preferences independently.

13. In-app notifications: Provider messages and results in-app are clinical, unaffected.

14. Provider messages: Clinical communications part of medical record, unaffected.

15. Insurance communications: Enrollment, premiums, claims, renewals continue for policy maintenance.

16. Security alerts: Breach notifications, suspicious activity, password resets cannot be unsubscribed.

17. Surveys stop: Post-appointment surveys cease. Feedback welcome via dashboard.

18. Support: Emails continuing after 10 days: support@healthtreatsecure.info or +1 804-268-2531.

19. Grievance: File with Privacy Officer. Acknowledged 5 days, resolved 30 days.

20. CAN-SPAM: Compliant with CAN-SPAM, HIPAA, CCPA/CPRA, state regulations.

21. Data rights: Access, correction, deletion, portability rights persist regardless of subscription status.

22. Verification: May send confirmation email to protect against unauthorized unsubscribes.

23. Preference center: Granular control — each category independently toggleable.

24. Special offers cease: May not be available upon re-subscription.

25. Seasonal campaigns cease: Flu, summer wellness, back-to-school emails stop.

26. Archives: Past newsletters accessible via website regardless of status.

27. Suppression list: Email flagged "do not contact — promotional" across all marketing systems.

28. No data sale: We never sell email addresses. Unsubscribing doesn't affect service-delivery data sharing.

29. Multi-email: Each address must be unsubscribed individually or managed in account settings.

30. Re-confirmation: May periodically ask subscribers to re-confirm. Non-response ≠ consent.

31. Effective date: Recorded in system and confirmation email. Used for dispute resolution.
